That happened to me once when fighting a Rathian...I was charging so I culd slam a 3rd charge while it was charging at me..(Using great sword at the time) and the lag thing happened and instead of me landing a charge on its head to stop it, it teleported right infront of my friend who was sharpening his Longsword XD

he was playing with a friend. the friend joins the terrain with rathian in first, he joins after. this is a bug i see every 2nd quest i do with my friend. the synchronisation is really bad in mhf, as u can see with the little monsters, that are on completely different places.
