I know the driver of the lorry personally, she is my Dad's partner. Very lucky to stil be alive after that. When anyone looked at the lorry afterwards they could not believe she got out in one piece with only cuts and bruises.

IMO it seems people are out of reach of eventual breaking equipment.
The load is obviously not dangerous unless you drown in it.
Maybe they should make sure the truck wouldn't roll (if the brakes are broken) but that's all I can come up with.
The driver obviously got out without too many blood stains in the cabin, so no problems there either.
