You missed two. The ladies and men's rooms both have different endings, each ending in your death.

Also, you really should consider fixing that audio delay...

That aside, not bad, I'm sure most people who didn't kill themselves every time they got the door right on their 1st or 2nd try the way I did will enjoy this. :P
