I sometimes put tilak on my knuckles and do 3 horizontal stripes (I'm Saivite) and then put the bhasma ontop of the tilak, and I don't put any on my back. I also do red kunkuma along with the tripundra.
All Glories to Sri Sri Guru and Sri Gauranga! Hare Krsna, I have never seen anyone put Tilak on the heart, it's meant to be on the chest. See pictures of Srila Bhaktivinoda Thakura or Srila Bhaktisiddhanta Saraswati Prabhupada they put Tilak on their chest.
