That "scratch" is the scar from when Kain was killed as a human in Blood Omen. He was impaled by a sword.
The necromancer, Mortanius gave him the chance to avenge himself and kill his killers. Kain accepted and Mortanius gave him life again. Not as a man, but as a Vampire.
I find it strange that Raziel has had a physical reaver that he remade up to this point, yet is able to wield it like a spectral reaver which then winds around yet another version of itself at this point.
so let me get this straight, if Raziel would have killed Kain now Kain's soul would have been sucked into the reaver and the hylden would have been freed and Raziel would have killed that wheel of fate squid monster like Kain in defiance?
@Scotty678966 Raziel refusing to kill kain with the Soul Reaver gave Kain the ability to save Raziel when he is about to go in the Reaver blade(the blade is before its the soul reaver) which in turns gives raziel to become the one who saves the vampires by purifying the scion of balance which in turns says they are both the Vampire and Hylden champions. Very Confusing but there it is.

In my opinion it is more likely the soul reaver would still have trapped Raziel in it, and then NO ONE would be around to save Nosgoth,
