after listening to the entire xfm recordings, i dont think this was a joke. there's just something about the vibe of this that is different than the programs. steve was genuinely offended here. and no, it wasn't karl having a go "one time", he did it all the time, more than steve to him i'd say. still, karl is human and like everyone crosses the line and hurts people he cares about.

clearly Karl was implying that Steve was an invalid in the sense of someone with a disability. Generally a freak has the same connotation. When he calls steve an invalid, there's no air of a joke to it, it's actually a serious comment.
