You need Natascha to perform the transformation ritual. Spin up Sasha in the spawn, hold down the spinning, then go to the console and type in equip 6 0 8, and when you equip Natascha, let go of the RMB.

If you are going very slow, you've done it right. Just don't use the Minigun, or the effect goes away, and you're a normal Heavy again. Enjoy the slowness and hilarity of Civilian Vlad.
