Lucas, I think you are the closest. During the 45+ race, I was grinding across those painted lines during some fast laps. That paint was slick. I think what happened is, the front wheel got loose, he overcorrected, then his tire caught when it exited the paint, he slid forward on the bike, unloading the rear wheel, lost the rear wheel and was done.
I can attest to the fact that the manhole cover was slippery (though I kept it upright unlike many others on the day), but this dude did hit the manhole cover. Not even close. It almost looked like his wheel slipped just as it came off the painted stripe, but I'm really not sure how he managed to slide out the back wheel.
Pause at 1:00...it looks like his front wheel may have clipped the back wheel of the rider ahead of him.

If you look closely and slow down the video, he never hit a manhole cover. It looks like he just locked up the rear wheel and overcooked it

he hit that slippery pothole cover. saw the same thing happen in 1-2 of the other races
