Bullet cam embedded into foam of crash helmet. Helmet cam and MIC need 12V. All cables have to be home made. 6 core cable goes from 4 pole/3.5mm jack (camcorder) to Video BNC (helmet cam) & Audio Phono (MIC). One conductor in AV cable for 12V to camera/MIC (via Phono plugs). XLR to 12v battery. It's not exactly lightweight, but tough. Wasn't trivial to solder together either. Angle of helmet cam critical!
See my Coed y Brenin video for my other setup - wireless stem camera!
If I was starting from scratch now, I'd probably go with RF Concepts - their stuff is nice, and you can get mini solid state storage camcorders now. I built my stuff over 6 years ago before you could buy anything. The only thing I bought 'off the shelf' is a LANC 'rec on/off' switch. Must get all my old footage on YouTube! - Good Luck!
Cheers for the memories, what camera setup did you use?

Some new ideas there, cheers. Been looking at mini DVR packages from RF Concepts but not completely decided at the moment.
