This a declaration of love to a woman (lover or child). It is a beautiful hyperbole, as the woman is described as his spiritual salvation.

To begin with, the persona signing is messed up: " the stuff I think is not allowed" and "the medication tries to do its part" and he is cut out of society. This woman "puts him back in the game". She is compared to an angel sent from God to heal him, and he praises God (Amazing grace is an allusion to the popular hymn) for his mercy.
