a game will never be realistic: If they would they would be boring.

Why? because each time you get shot you would have to spend 5 months in a hospital, you should do rehab, and if you would get killed it should say "Game over, you cannot play anymore". so no, there aren't realistic games.

Best-looking nuke I've seen in OFP or ArmA 1/2!

Needs adjusting though. The initial blast wave should be fast, then as the back-pressure sweeps all of the debris towards ground zero you get the dark ugly mushroom.

Some leveled buildings would be nice, too.

Used with dmarkwick's addon maybe (some of?) the trees could be made to catch fire?
