Thanks, yes about a year ago I really wanted to do (record) one for SH2 and was training for it (even I get rusty over the years and it's been 6 years since I did my first 10-Star run with SH2), but somehow I never really got to do it (shame I know). But don't worry, I will do it one day. ;)
You need to enable and set "Bullet adjust" to at least x5 in the extra options at the beginning of every "Next fear". It will multiply the amout of bullets you receive with every ammo pickup. For example, usually you get 6 shells of Shotgun ammo per box. With bullet adjust x5 you get 30 shells. If you haven't unlocked x5 or more yet then I suggest you do that first before even trying to get a 10 Star Ranking. You unlock a higher value every time you see an ending you haven't seen before.

2. How do you keep track of how many items you've collected and how many enemies you've killed? I'd think by using a counter, but you seem to never stop and even the pause menu doesn't stop the timer. Is it just by knowing the game well or do you have a special way?
1) Well, before I made this particular run I had made two 10-Star runs before, and I think I must have completed the game about >35 times before this, but the problem is that before I made this run I hadn't played SH1 for a about a year which isn't a good thing if wanna do a flawless run (and this run isn't falwless, though it's nothing that ruined the ranking). You can use notes so that you know where to go and what to pick up, learn to play without using the map and as soon as you manage to complete in under about 50 minutes you should start practicing battles. Try to find the right balance, know when it's better to run and when you have to fight.
2) You don't have to keep track. It's all about knowing when to fight and when to run. The rule of thumb is: "kill everything you see on your route, exept if you risk receiving a lot of damage, for example because there are too many enemies, or you don't know their position (Air Screamers)".
Items: easy, just pick up every item that is on your route and you're good. The trick is to know where they are. Don't care about items that are out of your way, you'd waste too much time on them.
The route is probably the most important factor. There is no way around it, you have to know the route by heart. You don't have the time to look on the map, and, as you already know, the game timer doesn't stop when pausing, so you even rarely have the time to read your own notes.

Yes, the route is very important, my suggestion is to play through the whole game in "speedrun style" which means don't kill monsters if possible but try get through the game as fast as possible in one session(!). That way you will learn the routes and the puzzles better and much faster. If you manage to complete in less than about 60 minutes, then you're ready to try a 10 Star Run.
No, I didn't upload this in high quality in the first place because it just would've taken too much time to upload such big files, so I dedcided to upload in a format which will result in standard quality. I deleted all video files, so there's no way I could upload it again in high quality.

Like it says on the ranking screen this took me 1h 21m 23s. But this is not a SpeedRun, this is a 10 Star Ranking Run so of course it takes longer to do such kind of a play-through.
I also made SpeedRuns (but never made a video of it) and my best run was just above 37 minutes (37m and 10s or something). I tried to beat the 35 minutes guy, but damn that was a perfect run that guy made so it's somewhere near to impossibility to beat him by more than a few seconds, if at all.
That 35:29 speedrun you're talking about is mine. It's actually very outdated and is easy to beat with the many tricks I have discovered since then. I've been planning on redoing the run nearly since I first uploaded it but never got around to it (I did return to the game to do a multi-segment run where I completed it in 33:50 with the good+ ending but I have never redone the single segment run). Perhaps one day I will; I could easily shave 2 minutes off my 35:29 run under the same settings.
Oh cool, you're the one that made that "legendary" speedrun. I mean everyone who is into SH1 has heard of that 35 minutes run! Anyway, I'd definitely love to see you shaving off 2 more minutes from your 35:29 run.
It's actually kinda hard for me to imagine that it is possible to shave THAT much time off your perfect-looking 35:29 run.
The problem with the 35:29 run was that inferior strategies and routes of that time were used. So despite the fact that certain parts of it might look "flawless" the very act of perfecting an imperfect route already hurts you. The run is very old now (recently having its 4th birthday) and I hope that at some point in the future I'll return to it. However you just never know with these things; the last multi-segment run I did took alot of desire to play this game out of me.

attempts. The original goal was doing it in under 1 hour but the last "practice" run I did was 1 hr and 1 min. I had just finished tweaking a few final touches to the route before losing interest (a common pitfal in speedrunning). From my practice sessions, if you take full advantage of the 2 saves you can use, I believe a 10 star run can be done in around 57 minutes. However, as of now, I have no plan on returning to SH for either a 10 star run or a Single segment speedrun.
Hey would you be doing any other 10 stars Silent Hill run ? I admire your skills and I'd be glad to watch you playing SH2 or 3 :D
