it is the carotid artery and this guy of girl had a very high cholesterol. now he has an atherosclertoic plaque so the blood couldn't reach the brain on this side, because the plaque was blocking everything. they removed it now.
probably it is a fat guy/girl, they have the highest chance of getting this
