When I try to go into my head voice my neck and jaw muscles get tense and it feels like my head is about to explode.

Brett Manning talked about the bridge between chest and head voice many times, his advice is to use the 'miaou' exercice to find the bridge and start connecting. I agree this video need a link to Brett's video to have a starting point, or there's no tip at all...
