Yes he is dead. After Assassin's Creed Revelations, they released Assassin's Creed Embers. Assassin's Creed Embers is a short computer animated film where it shows Ezio retired and is a father and husband. He dies of old age in the middle of a plaza in his birthplace of Florence while shopping with his wife and daughter. Sorry for spoiling that for anyone, didn't mean to.

Nostalgia...how I will never forget the first time I played Assassin's Creed 2. I actually felt like an Asssassin. And at the same time I could relate to Ezio, not at such a level that I lost my family, but that we all go through problems in life, we all have our insecurities, Revelations was great and tied off a lot of loose ends. But in my personal opinion. AC2 was my favorite one. Storyline, characters, being in Italia, and not to mention some of the most amazing music in a VG I have heard :)

When I first heard this song on the game, I knew the game was going to be great. Seeing Ezio grow up from being a teenage boy, into a wise man in Brotherhood and Revelations, made for an interesting series of games.
