Oh, I think I get it. So Sanchez is in love with the girl so his world is turned upside down by not being with her and the blood symbols the pain he's feeling. Then when he refers to the hole in your head it's referring to the image or her and him that he keeps staring at. He's trying to erase her but she's still a constant in her mind and heart. Poor Sanchez, but I still love how beautiful the song is. Go PHANTOM
