@dparnell89 I prefer hand-sewing because I can use the technique in more situations, with more varied needles & suture materials, at lower cost, and with greater joy in the art of suturing. The focus of this video was the repair of the gastric injury, but I do use 0 Ethibond for hiatal sutures, reinforcing with absorbable mesh for large or redo hernias.
@BCUboi This is a seromuscular tear of the cardia of the stomach I created while dissecting out a recurrent hiatal hernia. The ischemic-looking piece of fat peeking in from the left is the excised hernia sac I eventually removed and discarded.
i learnt two important points from you. first, the idea of leaving the needle in the left grasper in order not to loose the needle. second pushing the tissue with the back of the needle to make the suture tight. i practiced this and found it very helpful. THNX AGAIN.
@wuzhig I use a Storz Szabo-Berci needle driver set (Parrot jaw on right, Flamingo jaw on left). Their curve helps me handle the suture and tie more easily.

@ahmedraoaf That is actually a 2-0 Vicryl on an SH needle. I use Ethibond for hiatal reconstruction and fundoplications but prefer an absorbable material for repairs like this. This example is an undyed Vicryl, but I typically use a dyed one because it's easier to see inside the abdomen. Thank you for your feedback.
That's a big Hernia, do you Endostitch it or hand sew it with 0 Ethibond? I've worked with several who have done both.
