How To Write a Review On Google Places / Maps

When I signed up for reviews I forgot to add my name or a picture. Now it posts without either and I want my name to show. How do I change that?

After I write a review and log out of my Google account, the next time I log in, how do I find a list of reviews I've made? I know one way is to find a review you've made and click on your name, but what if you can't remember where you left any reviews? Thanks, nice video :-)
