The beginning is like the "welcome to Hell" portion of the song. You know you're getting closer and the fights are becoming much tougher.
The drumming part makes me remember fondly of the black knights and Warlord of Blood coming to slaughter me.
And the last minute is that sudden realization that you're about to come face-to-face with the Devil himself. Will it be this staircase? Or the next? It was ominous, mysterious, and twisted.
Sad that Diablo 2 failed to bring back the horror of Diablo. Diablo 3 probably won't get any closer either, though it seems it will have an "epic" tone and crazy good atmosphere... just not the true horror this game had.
As you descended the levels it gradually got darker, scarier, and harder. I actually felt like I was in the thick of evil while playing. I felt like if I stayed too long I could be corrupted and go insane.

The soundtrack for diablo 1 is absolutely amazing. I don't like the path blizzard took with this game. The second one is an awesome gaming experience, and I am sure the 3rd one will be great too. But since Diablo 1 nothing that blizzard developed has managed to give you goose bumps like the first diablo did.
the difference between D1 and D3 is more than 10 years, I first played D1 when I was 8 and it scared me enough to play with the volume lowered or off
now I'm 19, I played D1 again just for memories with normal volume and I only jumped like once or twice during the entire game and its hardly scary for me

The Hellfire-only music was still there though. I'm 5 years old. Cut to "Hive", cut to music, cut to the freaky text at the beginning, cut to my first encounter with "The Shredded". I darted out like a bat out of hell. Even after a massive level-grinding, I was still afraid of that music. I got used to it, though.
