I think the last verse is what he must have imagined she was seeing as she passed from here to heaven. It is dedicated to a girl who passed away with a cancer. A very close friend of him. The theme basically describes the last moments with her (little Gem) and Van feelings.
The anxious breathe, never wonder why, it is simply this way that must be. Little Gem is gone... (sorry for my bad english)

Ok, I'll admit it, at first, the only reason why I decided to listen to this song was because it was Robert Pattinson's favorite song.
But, now, (honestly) it's one of the most amazing songs I've ever heard, the deepness of it is incredible.

This has a bare-boned complexity to it whereas the album version is very tranquil.
This one is better to me but still I would be hard pressed to find another better than the album version.. He's that good!
