Maybee after Radionucleide thyroid scan when it shows hypofunction we might need to do an US, if is suspicious of malignany then do FNA. Your diagram makes more sense to me but this was updated lately in one of usmleworld tests.

I would like to make two comments:
a) It would be nice if your diagram will show how thyroid cysts are managed compared to the solid nodules;
b) The biopsy result should also include non-diagnostic [inadequate specimen] or indeterminate [neoplasm: adenoma vs. carcinoma]
