I've had this happen on a spinner with two trebel hooks. My dad had a great fishing spot where the rock bass always hit hard. On a good day, when they're really hungry and fierce, they will fight over the bait and both get a mouthful.

I've caught two bass on the same topwater lure several times but the most unusual thing that ever happened to me is when one speckled trout took two lures on two different rods at the same time trolling, both lures tightly hooked up in his mouth.
