Ask for the 'long form' birth certificate form. It is also called the 'certified copy of birth registration'. It is actually the same form to request your other birth certificate (the birth certificate request form). There's three options on the form for a request; check the long form one. I found it so hard to find, but so glad that I did.

I am a strong supporter of peoples rights and privacy and private property for that matter and I respect this movement for standing up for it, however I am an athiest. I am 99.999999% certain that no one chose to come into existance.
