In the Benezia boss fight the same thing happened to me, except all the boxes flew away completely and i had no cover, my last save was 10 minutes earlier :(

Tetra, you have to make your team mates use the weapons that they are trained with otherwise the weapons that they use are ineffective and they are useless to you. For example Tali cannot use sniper rifles or assault rifles, only pistols and you can train her to use shotguns. Liara can only use pistols so make sure she has a good pistol. You should also tactically use your powerups as you don't seem to use them at all, it helps alot and you won't die as much. But I love your lets' plays!
