I think the grandmother has valid reasons. The little boy sounds as if he has become fearful and untrusting and has had some personality changes since the boyfriend has entered his life. How can this woman believe a 3 year old over grown men/women? Children do deserve to be protected at all cost, however, children tend to blow things out of proportion. If you child says they have been abused or neglected, you investigate once and if the case evidence can't be found, then you drop it. Children change and go through changes in behavior. So this lady needs to let this go.
