Its a Epic Helm for warlocks only, that will randomly show a set of wings come out and flap a couple times before disappearing. I believe its a drop from a 10 man nax raid.

no, its from burning crusade expansion, the T6 Helm, "of the Malefic" from Archimonde in Hyjal raid, or season 3 arena sets. Both helms have the same effect, just different skins/colours.
