MAD only worked when both sides saw a stark detriment to launching a strike, i.e. both sides were sane. The US will not launch a first strike against NK as a matter of policy. However. I do fear that Kim Jong-il, with a nuclear bomb, may just decide at some point to use it on Seoul or Tokyo if he feels he's not getting what he wants, i.e. attention or more extorted aid, and has little or nothing to lose.
