we did see a white sleeve and well we don't see Alex after the accident so it may be him , i didn't see the picture but i'm 70% sure it's him .

I think that Alex is hit by the ambulance. In the promo pictures we can see Ava and Lexie looking to an operation and I thik is Alex.
