Most people have little or no pain in their deep wounds, because the nerve endings are destroyed. They feel nothing during maggot therapy (MDT). Some people have painful wounds, with pain during any dressing change. They will likely have pain during MDT, when the maggots become large enough to feel them: at about 24 hours or half-way into the treatment. If pain meds are not adequate, the maggots can be removed a few hrs early and all discomfort ceases immediately.
It is standard practice to wear gloves for both nurse and patient safety,A persons hands have bacteria on no matter if good hand washing is used. The last thing you would want to introduce into a wound is mrsa as a large number of us carry it on our skin with no ill effects. Im dissapointed that such a major mistake was made by these health professionals. I am a student nurse an seeing such poor practice shown across the internet worries me it will lead others to believe it is acceptable.
@mmnusteph Maggots consume potentially harmful bacteria as well as dead tissue, so I don't understand why she put gloves on at all. The amazing result of what they eat is allantoin, when it comes out the other end. It's the "magic" ingredient, treasured for centuries, in comfrey roots.
@Chazfem Yah, the whole neighbourhood and the dog could be there, providing they don't interfere with stupid remarks or get in the way of the person applying the dressing and maggots.
Maggots like bacteria as much as they do dead flesh, and they pig on them as they go along.
Oh noes he's ill, help! don't worry, I've been to the morgue and collected some maggots from a dead rapist, we'll pop these inside your husband and he'll be fine by next week. Wha?
1. Gloves were not used in the beginning (use some gloves eesh eww)
2. The brush from the contact cement looks like it was used over and over again. It should be a one time disposable brush. Or your just a) incubating gods knows what you've picked up on the brush and put into the glue and b) giving gods knows what to everyone else you use that brush on.
Gag.
Other wise everything else looks great. I'm all for maggot therapy.
By the way what did the gloves touch to get any germs or spread any germs? If glue is used just for gluing stocking on? I had maggot therapy done the same way, with NO problems. No gloves needed if not touching wound or anything that touched wound. Or if not touching maggots. Afterwards believe me gloves are used to remove the maggots, as then they have the patients bacteria.
I Usually do not argue with anyone! Hate too see wrong info get out there I see it all the time. Usually by doctors who have aliitle info about MDT and not enough!!! My postion is the number one patient advocate in the wrold for maggot therapy!! I try to educate patients and docs to do the research and learn how well maggots can work and all they can work on. Always worth a try! Sorry came across too tough! Just trying to do my job! : )Tell Ron (Sherman)Hi!
ANYONE can do maggot therapy for a patient or themselves with a doctors order and follow-up. Yes would love to see more docs do it themselves but they are too busy or squeamish themselves. It is always worth a try no side effects and it is cheap. Unless a doc charges you a fortune to apply!!!
IF you notice she never touched the wound with ungloved hand. I guess you need to learn more about MDT! PLUS to top it off it is a demonstration and NOT a real wound..
Not sure why the technician was not wearing gloves through the ENTIRE procedure. Not only does the patient's risk of infection increase, but I am sure whatever caused this wound would be unwelcome in the life of the technician. Very, very poor standard of care. This video should be tossed for this alone. On another note, I'm not sure what the two idiots at the door thought they were doing "peering" in on a delicate procedure, but they should both be given the sack. This video belongs in 1985.
I agree! It's disgusting that proper hygiene is not practiced in the health care setting. Many people die from hospital based infection, each year. This video should go.
well, considering the wound is rotten flesh, the most important part is that she not infect the maggot since they were sterilized. she did not touch the areas she sterilized and did not really touch the adhesive surface of the hydrocolloid barrier. it could be worse-they could just let him rot :?
ACTUALLY she never touched the wound with ungloved hand. What she touched was not areas that needed to be sterile. AND if you look close the wound is a demonstration and is NOT real!!!
Lots of federal mandates in place (not including common sense) that require practitioners of medicine to wear protective implements during ALL medical procedures. I think it is especially important here; she is working with an open wound and biogenic maggots. That she never touched the wound is irrelevant and moot. She is breaking the rules. I dont care if the wound is fake or if this dude has a necrotic leg hanging off its bone. Demonstrations SHOW us how things SHOULD be done. She messed up.
FIRST of all I know you do not work in wound care, You'll notice no wound care specialists will comment on lack of gloves. It is IMPOSSIBLE to lay dueoderm with gloves on. As long as wound is not touched for nurses sake and hands washed througly. It is fine. Do you really think doctors wear glove during entire surgery???BESIDES this is NOT surgery. Patients can do at home themselves. It is not rocket science. Do your research and learn more about MDT!
I really dislike squabbling with strangers. Makes my eye twitch. So I am going to end this quick, fast, and in a hurry. I do not work in wound care. I'm not going to notice that "no wound care specialists will comment on a lack of gloves" because that would put in place a precognition that everyone who commented was a specialist. That is just silly. I don't know what is possible and what isn't. So I did do my research for the sake of argument. I walked my ass to one of my local hospitals.
When I got there, I spoke with the only doctor in my city that uses maggot therapy. He is an internist that specializes in infectious disease. It was a good talk. I am just going to address a couple points here. Do I really think doctors wear gloves during an entire procedure? Yes I do. If that doctor isn't, he is being naughty and can be sued. Do I think this is surgery? No. Does that mitigate the need for a practitioner to employ the standard of care? It does not under any circumstance.
Can patients perform this procedure at home? Yes, I suppose they can. Can they do it legally and properly? No. Medical maggots are special maggots that cannot be "prescribed" as painkillers can be prescribed. Moreover, it is illegal to carry, transport, and use them unless you are a medical researcher, a doctor, or a trained specialist in wound ostomy care. Apparently it cannot be done at home because science is involved, even if rockets are not. So I will conclude with what the doctor said.
I am sorry but his info is not correct. I went to the top man in the field. Dr Ronald Sherman "The world renown expert in maggot therapy" Where he might agree to some things, most of this is I am sorry "crap" ANYONE can do the therapy it is not illegal. patients do themselves and bill Medicare for them which pays. I think this doc dropped the ball. You may need to apologize now
I inquired about the use of gloves. I asked him if he could think of any circumstances under which gloves would not be worn during the course of the procedure. His answer was no. He noted that if the therapy was being given in a third-world country under conditions where gloves were finite in medical care, that they made not be used. He also noted that they (gloves) are essential in cases such as these for a few important reasons, not the least of which is because it is required by law.
Fed Ex delivers them to your home. I myself have done maggot therapy for a person, ready to loose her foot. Patients have done themselves to save their lives and feet. I wonder if the doctor has ever actually put on the maggots himself or had a nurse do it.
So those are the opinions of a practitioner of 47 years. You might not think it important because this was a demo or because it isn't an invasive procedure. Those are piss poor excuses to defend laziness and sloppiness. I wonder that if I took a poll, everyone would be quick to jump on "no gloves?" I am going to err on the side of people's desire for safety in the medical profession. Sorry we disagree. But it is good to know I was coming from the right side from the beginning. Case closed.
Not going to argue anymore either. Yyour doctor came across from what HE knows and has to say as a doctor. Maggots have to have a prescription but anyone and I mean anyone can do themselves at home. Ha Ha Where did he get those ideas of transporter them??
I think it is funny how you brazenly state that the information coming from a doctor with tenure of 47 years is just wrong. I am finished with this ridiculous discussion, but I am going to make sure I get all the information I can. I am going to confer with this Dr. Sherman and get his take. Then I am going to do what I do and read up on the legalities. This is for me, so you can move along to your next diatribe without any apology, for to offer one would imply that I had done something wrong.
Yea I've seen this done. The same place has the only clinical trial of using bacteriophage to heal non-healing wounds like diabetic foot ulcers and pressure ulcers. It's pretty crazy!
no freaking way i.d do that ! XD

1. You risk exposing yourself to whatever is responsible for the wound, typically an infection that will not heal on its own.
2. You risk exposing the fragile maggots to infection, which would render them useless.
3. You risk making the infection worse by exposing it to hand-borne germs etc.
I asked him what would happen if he caught someone "ungloved." He told me that they would be "disciplined" and could face patient redress.
