i disagree, the bar you use is smal so its easy to lean forward and lay thebar down or extend your arms back and lay it on the floor, or there is specific benhes with a litle plaform to put it on.
i dont really like lowering to the top of my forehead, the elbows comes under alot of pressure, also the long head of the triceps is not included in that form.
I stop the bar around the back of my head or simila, letting the upper arms going a bit backwards
Elbows in, you have to keep them as close as you can. You have to also try to keep the elbows in the same position throughout the whole set. Don't move them back and forth. It's a nice exercises, I tried it a couple of days ago. And like the video says, start with a light weight until you get the movement right.

By far my favourite tricep exercise. I've gotten great results by keeping the form, not heavy weight. Remember...Form > Weight!

this is a good tricep workout..only prob is for heavy weights a spotter is required...also the form is really important.

How far should your hands be apart?
Shoulder distance? Or an extended thumb gap?

Around the head provides better range of motion.

elbows in or out?

You want to make sure you keep your elbows in...not flailing about. It'll put more stress on your triceps.
