The singer (the addict) is telling the tale to his addiction of how he lost some girl that he loves due to said addiction, he repeats "Always" to the addiction, he's torn between the girl and the addiction. The addiction talks back (that's the whole "pistol shaking in my hands segment.") He also talks to the girl some ("I left my head around your heart"). Great song.
