That was from a dream sequence in Maternity Leave, in season 1. And it was actually one black eye and one white eye.

It kind of bothers me that John isn't as mysterious as he was in Season 1 anymore. Back then he seemed like a much creepier character and more knowledable of the island than even the man in black.
I hope Terry O'Quinn brings back more of this through fake Locke, who there is absolutely no reason to show any 'humanity' through now that his jig is up.
