This is I guess a rare malformation but it's more common to see in countries of the 3rd world where there is no pregnancy follow up.

What does the baby's inside anatomy look like ? Did the right lung look like the left one with 3 lobes ? were the large vessels of the heart sticking out ? was there any pericardium ?

What were the principal causes of death ? (I can think of a lot)

Sometimes its just bad luck - it can happen to anybody but, most of the times it is because Indians tend to marry and have babies with their own family for example, cousins marrying cousins. This can cause a mix up of the same DNA which can lead to problems, like what you have just seen and disabilities... etc :) That is not always the problem it can just be bad luck as i said. x
