Its not speed factor. Two trains on same track cannot be issued authority to proceed, in opposite direction. Shuntting operation is also not allowed on passenger track. Modern Railway System dont allow other train when one track has been given line clear. In my opinion, some portion of track between 2 trains was not circuited which allowed points to be set for opposite trains on same track.

@JogBird the faster it gets the harder it is to stop it. implying that the train was out of control otherwise it is a human error, trains are often on the same line but at far distance and switch on others rails at certain points to avoid collision but if the operator who tells and guides the trains is wrong then collision can happen and difference in actual speed and speed known to operator is a factor
