ive only had that happen to me while i was playing it on my 360.

that stuff happened to my character but one time when i did it his skin ripped off and you could see his veins and flesh and internal organs and brains im just like "Well...okay then..."

I had those leaves on my face, and the eyes of my hero occasionally turn yellow or disappear.
