I reckon i've got a small insight into what the songs about: if you are smoking outside a hospital, you are walking away from what is best (the hospital), or ignoring it for your own pleasure or enjoyment, and to be happy (the smoking), rather than do the best thing.

"smokers outside the hospital doors" depicts those with loved ones inside suffering whilst the person in question is outside as they can't handle the situation in hand, and "say goodbye to everyone you've ever known" is something you would say to someone in the process of dying so that they have no regrets and can redeem what they had with those they love.

all in all, this is a pretty powerfull song with a great video, long live the editors.
