It looks like this arrangement may be the one by Lorie Line as mentioned previously... The book containing it is her "Simply Grand" one. It's not cheap but I've ordered one... hopefully it's the correct one as the arrangement in the Zbigniew Preisner book is by Martha Kent and I really don't like it much at all :(

I think the sheet music for this version of the song may be contained in the book "Zbigniew Preisner: The Piano Collection" (it's on the list as a piano solo version, at least). It's sort of expensive, though :/... wish I could buy just the one song. It doesn't seem to be available on any online digital-distribution-type sheet music stores.
