She's having a sexual relationship with Danny, which is pretty weird. And Meredith is screaming because Derek is dying, which turns out to be a dream at the end of the episode.

The title for the season premiere is "Dream a little dream".

So the whole premiere is obviously all about what ifs. Dreams vs reality, or rather... nightmares vs reality.
