How could someone even think that he DOESN'T care about Izzie? I mean, come on, he can be a jackass, he totally can, but he's always been sweet to Izzie. And he's always loved her and done everything he can to protect her and be there for her. And sure, he's made mistakes along the way, like sleeping with Olivia, but he's totally made up for that by standing by Izzie when she had cancer. So... Alex does love Izzie. No debate needed.

More scenes could be the one in season 4 where Izzie is crying and he brings in the tissues, his reaction to finding out she slept with George, the season 4 finale where he kisses her, season 5 finale when he covers her with the blanket, in season 5 when in the clinic talking to that guy about when to say 'hello' to someone you like, the 'I care about you scene' and all the scenes with depressed Ava and how he takes care of her. There are so many examples of him caring.
He totally does care for Izzie. He always has. He uses his abruptness as a defense mechanism. He doesn't trust people, which could be attributed to his childhood. But he trusts Izzie, I think she is probably one of the only people he has let all the way in. He cares, he just has trouble showing it. We saw this in the most recent episode about them not dating other people. He cares, he just doesn't know how to show Izzie that he does, that's why he asked for lessons.
