The girl is missing her eyes and seems like a puppet of sorts to create an uncanny-valley feeling, she has what resembles a weapon in her hands, there are hands coming from behind the door (or whatever) behind them, their bodies give out vibes of mal-intent, and the dark lucidity of the painting conveys horror.

It's pretty easy to tell why it would creep people out, it was obviously intended to be scary.

@Demanga thats not a weapon its a battery, the hands are children on "the other side", the girl is a doll which she is his guide threw the "the other side", the door is like a layer between reality and "the other side" and the boy is actually the artist of the painting as a boy, i did some research on the painting and the artist paints very creepy things he even made a future version of this with himself as an old man, sorry i just love talking about this painting ^ ^
