Anonohio's comment about the incidence of intraoperative awareness is only off by a factor of about 500. The true rate in the US is one to two per 1,000 anesthetics. (Sebel et al Anesthesia and Analgesia 2004)
@ jacobkln: We just trialed the BIS. It was interesting to note that the reps selling point to us was that we could use LESS anesthesia, and therefore save money, because we could "see" where the patient was at. quite a bit different than the line they're selling the public.
Oh, as far as the incidence - Morgan/Mikhail, Barash, Stoelting (who have all put out anesthesia bibles) all put awareness at 1:250k. take that how you will. MAC cases don't count, I'm sorry.
Oh, one more thing. Aspect has put out a few studies saying that anesthetists actually use TOO much anesthesia, to back up their selling point (like i mentioned below). Bottom line: they're trying to sell a product, and they're telling their 2 target consumer groups 2 different lines.

But BIS is about a more than just awareness. The goal is to give the patient the optimal dose of anesthesia; not too much (which can lead to extended PACU time and vomiting and such), or too little (which isn't good for obvious reasons).
Not saying BIS is the end all and be-all, but it can be a very valuable tool.
thank god its invented... i saw the movie "Awake" and was astonished to find out that this is a real phenomenon ... and i was concerned but this is good stuff....
when I was 6 or 7 I was under Anesthesia only once cause I was getting 2 front teeth pulled out at the Children's Hospital and I was hurting a lot during my stay in the recovery room
I had 3 general anesthesia I was unconscious but nothing happen to me.

...........oooook: anesthesia awareness is almost unheard of EXCEPT in 2 situations:
1) emergency c-sections
2) emergency traumas
in both those cases to put the patient under too deep might kill them, thus there is a higher incidence of awareness
in general surgery, i believe it's somewhere in the range of 1:300,000
in other words, insanely unlikely.
educate yourselves, people. don't get your facts from hollywood.

I've seen a brain dead patient with a BIS of 55! Not a very reliable tool but nice to have.
