you need to kill something called a shmoo until it drops it, its in the inverted library.

In the first video he initiates a conversation with the librarian before the poison mushroom kills him. In the other vids he is exploiting the use of axe armor to transform into an axe knight while under the teleportation effect of the fireball spell.
