Should have just used a Balloon Sinuplasty TM catheter and irrigated.

That hole is barbaric and will lead to over ventilation and scarring.

It's a surgery, not a movie. Most endoscopic surgeries appear this way because the doctor is constantly re-adjusting the camera to a better angle.
