The very best are at the top because they do an unbelievable amount of work with no time off. When Phelps made his big splash in 2004 up until then the longest amount of time he'd been out of the water was 2 weeks. Pat Mendez Snatched 207kg. Even injured he takes no time off. Squats instead.
