Two Boeing 737´s crashed because of this reason before Boeing found the problem, they didnt completely fix it but made it so that it happens much more rarely even in bad conditions, and trained pilots to handle it.

Actually third Boeing 737 almost crashed because of this, but they had luck and they landed safely, thats where Boeing engineers finally found the problem.

If only they had tried using left rudder inputs. All that happened was a reversal of input versus actuation. After the failure, they were holding the right rudder which was commanding left rudder movement. The left rudder pedal would have commanded right rudder movement. Oh well. Hindsight is always 20/20.
This accident was caused by temperature variations in the hydraulic fluid in the rudder acuator. the plane descended rapidly from flight levels, where the temp's were sub zero to temperate altitudes. The fluid going into the actuator was much warmer, and the sudden change made the rudder get shifted full left. This was a terrible tragedy, which also occurred in a United 737 landing at Coloarado Springs. The problem has been corrected.
I lived not even 10 miles from there. My grandma called and said there was the loudest BOOM...she thought Hills (which became Ames, and now Big Lots) exploded that it was so loud. I remembered hearing tons of sirens. I lived in Hookstown at the time.

The failure is contributed to the Rudder PCU "Dual Servo Valve". I worked at a Repair Station that was involved at the time with helping the FAA and Boeing to functional test and identify the cause. It was found that if the locking feature of the dual servo valve retaining nut failed, due to improper installation, the nut would loosen and back-off. When the nut had backed-off far enough, the dual servo valve failure could be duplicated.
To properly install and lock the retaining nut into position a special tool owned by (Parker Hannifin) the manufacturer was required. I suspect that many third-party repair facilities were unable to purchase this special purpose tool, due to propriety rights. This lead to improvising the tool. I know that using the wrong tool caused the largest single FAA recall of 737 Rudder PCU's for one Repair Station, the one I worked for.
Finally the FAA and Manufacturer decided to redesign the Rudder PCU, demanding a dual system in place of just the single system for redundancy and safety. There were countless Service Bulletins and Airworthiness Directives issued over the years, making it even more confusing. We have to be thankful that more aircraft and human lives were not lost because of this nightmare, and pray that this will never happen again.

The full answer is kind of complicated, but essentially a hydraulic valve malfunctioned and when the pilot tried to correct for it, it got worse - making the plane do the opposite of what he wanted.
There was no chance to correct for it since the valve malfunctioned in such a way that the pilot could not known what was happening. There is a book titled "The Mystery of Flight 427: Inside a Crash Investigation" that is quite good and explains the problem in detail.
the us airways plane went through another planes jetaway which is the smoke that the plane leaves behind similar to when you have smoke come out of your breath on a cold day. Well the US Airways flight got into the wake and got one wave of turbulance and then the plane rapidly tilted to the left and went straight down. It had something to due with the PCU(power control unit) in the rudder assembly. This malfunction caused the rudder surface to deflect in the opposite direction to that directed by the pilot, putting the aircraft into a spiral dive.
This also happened to another 737 in Colorado in 1991. The NTSB investigation lasted five years, the longest running in history. Their preliminary report stated initially that it was unsolved until they found out the cause of the accidents after a third 737 had this PCU malfuction, but the pilot managed to land the plane safely.

Lives Killed: Boeing 737: 3272 in 115 accidents. Airbus A318-19-20: 618 in 17 ones. Majority by pilot´s fault when aproximating a runway.

Air Accidents Investigation Branch recommended Boeing in 2003 after the Helios 737 run out of oxigen and presurization...to change the wire backup of the air conditioning-presourization valve in all 737´s.

The simple fact that your father was on that flight is *precisely* why people should talk about it. Anything that can come of the dialog to make future passengers safer is the ultimate goal, don't you think? Then perhaps your father would still be with us.

It was because of a "rudder hard-over" which means that the rudder turned without order to the left and the pilots could not overpower the turn or dive. My prayers are with the families, R.I.P.

most aircrashes could have been prevented.
many people have died needlessly in aircrashes, for example: 9/11, US Air Flight 427, Alaska Airlines Flight 261, Delta Airlines Flight 1141, and many,many more

Many may vouch for the safety of the 737, but there is something captivating about the ominous hidden flaw that the old servo valves used to have before they were replaced. This crash and the Colorado Springs crash are just so haunting because they went unsolved for so long due to the fact that the PCU servo valve flaw was so hidden. I was afraid of 737s for years until they retrofitted all rudders of the fleet.

We´ve got a new point here...
Air Accidents Investigation Branch recommended Boeing in 2003 after the Helios 737 run out of oxigen and presurization...to change the wire backup of the air conditioning-presourization valve in all 737´s.
Boeing currently hasn´t responded yet...so:
Each Boeing 737 is just a potential Killing Machine by lazyness of Boeing to change their systems...saving cost???IOL.

YEP...then we consider that the 737 has created more deads thab the whole Airbus Fleet in their service history:
The Killer Boeing 737: 4.779 killed.
Airbus: 2.018 deads.
PD: 3.272 killed by the Boeing 737 since first A320 flight ...IOL...still ahead of Airbus totals...

There are four times as many Boeing Aircraft in service worldwide compared to Airbus and the 737 is a large percentage with over 5000 737s delivered. Do the math...all types of planes crash, there are significantly more Boeing 737s along with others that their Airbus counterparts. If you are going to make a comment, make an intelligent one.
