Ok proficiency I took for wash mittens, apply clean gloves, remove old dressing and assess to what level the previous dressing was packed because the care plan was erased to an ineligible mess. talk, comfort console client, help them become familiar with the wound theyre always curious and they always know what the prior nurses have been doing. discard gloves with dressing inside, assess. wash mittens, clean gloves, set up sterile environment, if you are careful use a finger gently to pack.
You forgot to ID your patient and draw &close the privacy curtain. Hopefully you medicated for pain 20 to 30 minutes prior to dressing change. What is the consistency, amount and color of your drainage. Is there any odor. Whats the skin surrounding the wound feel like. Is there increased warmth. Always check for skin temperature at wound site using back of hand or tip of fingers. Tear off tape strips, time & initial them, and open & set up sterile supplies before removing old dressing.
@dbrooks12009 . those are very good points! however, set up of sterile dressing should be done after removing the old dressing. less exposure less contamination.

thanks this is refreshing I have an exam on this on thursday, can use all the help i can get, we will have cauterization and iv maintenance.
