Maybe this is some sort of engine failure testing? It looks like they purposely overloaded the engine to see what would fail. You can tell the engine had a sudden heavy load put on it. If the dyno had failed, it would abort and not continue full throttle with full load...and the operators would have pulled the throttle back, there was plenty of time.
i'm guessing the engine dyno failed. Keep your eye on the back of the engine that is the hydrallic pressure plate pressing on the fly wheel to keep the engine under load. When that failed the engine over ran itself and threw a rod.
If I had to guess? googled up some specs and it's likely this is an MTU/Detroit 8V396. There's a little lever in the background that drops as the engine starts to go south... governor maybe? The first thing that seems to happen is that the pressure hose from the turbo outlet blows... I wonder if the turbo partly seized up and started blowing chunks, or an intake valve seized up and caused overpressure/blew out the tube and then eventually dropped and jammed in the cylinder... hard to tell.
You figure, loud dyno cell, the operators are outside the room, with the engine spinning at high RPM, it would've been very difficult to stop. Being around stuff liek this before, what seems like a long time on video is a blink of an eye in the moment.

i agree with a failure test. also appears to be a commercial diesel engine. there are torque sensors on all four engine mounts.
