The thing is Makoto is a giant selfish perverted asshole. He liked Kotonoha, the only reason he went with Sekai over Kotonoha was because Sekai was willing to fuck him and be more open early while Kotonoha was more shy and reserved about all of that until the end when she was able to confess everything outright which was hard for her to do early on After her confession Makota even apologizes saying he never knew thats how she felt. Basicially it comes down to Makota not willing to wait. for her.

Sekai knew what she was doing, she knew it was wrong, and after a certain point, she didn't care anymore, she just wanted to be Makoto's girlfriend. She even admits it in the last episode before Kotonoha kills her. If anything, Setsuna was the cause as she originally was assigned to sit next to Makoto, but switched seats with Sekai.

That's partly correct. Sekai practically throws herself on Makoto and starts giving into his desires, because she begins to want him too. As a result, he begins thinking that fulfilling his desires is more important than fulfilling his relationship needs, and starts thinking that way about every other girl around him. Kotonoha was forgotten because of Makoto's newfound desire for lusty satisfaction, but to his credit it wasn't just his fault, it was Sekai's too.
