They rehearsed the scene countless times, because they obviously only had one chance to get it right. Christopher Nolan was very pleased that Heath didnt even flinch even as pieces from the explosion were being thrown at him. So yes, the scene really has heath in it. This can all be found out on the DVD extras.

If you look closely, at 0.50 at the left side of the screen, the bus from the scene (supposedly containing Ledger and the passengers) crosses the screen. That all proves it was shot in one take.
If you look very carefully, on the sidewalk/road with the white van on it next to the building on the left side of the screen, you can see Heath Ledger walking out of the shot.
Heath was an incredible actor. I read somewhere that the detonator was set to blow up the building on his first press, but when he turned around he saw there was no explosion he improvised by playing with the remote until it blew up.
They did the city a favor. It used to be the Brach's Candy Company administration building, and has been vacant since 2003. It is good it was taken down. google brach;s candy chicago for more.
