The punchline of the joke isn't even meant to be funny. People that are aristocrats are usually very sophisticated and refined people, the cultural elite of a society. So it's ironic that a family doing all of these vile and disgusting acts to each other would refer to themselves as 'The Aristocrats'. Like I said, the punchline isn't supposed to be the funny part, it's an inside joke among comedians as to who can come up with the most disgusting stuff in between the opening and the punchline
